Healthspan vs. Lifespan: What's the Difference?

Lifespan is how long you live, in total years. Healthspan is how many of those years you spend free from serious chronic disease or disability. The two numbers are not the same — and the gap between them is widening.

According to WHO 2021 data, Canada's Health-Adjusted Life Expectancy (HALE) is 68.7 years for males and 70.9 years for females — compared to total life expectancy of 80.0 and 84.1 years (Statistics Canada, 2020–2022). This leaves a gap of 11–13 years spent in diminished health.

The 7 Domains That Shape Your Healthspan

Decades of epidemiological research have identified the modifiable factors with the largest impact on healthy life years.

  • Physical Activity: Ding et al. (Lancet, 2016) found that insufficient physical activity is responsible for 5.3 million premature deaths per year globally. Moving from sedentary to 150 minutes of moderate activity per week is associated with ~1.5–3 additional years of healthy life.
  • Diet Quality: Li et al. (Circulation, 2018) estimated that adherence to a Mediterranean-style diet is associated with approximately 3 additional years of life expectancy at age 50.
  • Sleep: Cappuccio et al. (Sleep, 2010) found that short sleep duration (<6 hours) is associated with a 12% increase in all-cause mortality risk. Optimal sleep duration is 7–8 hours for most adults.
  • Smoking: Doll et al.'s 50-year British Doctors Study found that smoking costs roughly 10 years of life expectancy on average.
  • Alcohol: The Global Burden of Disease (2016) study found no safe level of alcohol for overall health, though light drinking (under 7 drinks/week) carries minimal measurable healthspan impact.
  • Body Weight: Bhaskaran et al. (Lancet, 2018) found that both underweight and obesity are associated with shorter survival.
  • Social Connection & Stress: Holt-Lunstad et al. (PLOS Medicine, 2010) meta-analysis of 148 studies found that strong social relationships increase odds of survival by 50% — comparable to quitting smoking.
